CARE COORDINATION SUPPORT SPECIALIST
BASIC PURPOSE/SCOPE:
Provide support for Comprehensive Care Coordination services for older individuals, their family, and/or caregivers as defined by the program. To enhance an individual’s well-being and promote their independence to the greatest extent possible.
PRINCIPAL RESPONSIBILITIES:
1. Comprehensive Care Coordination Supportive Tasks.
Assist Care Coordinators with management of comprehensive care coordination caseloads including but not limited to:
Scheduling appointments for assessments with participants.
Addressing participant phone calls and assisting in resolution of service- related issues.
Completing financial verification for waiver eligibility determination.
Ensuring Medicaid compliance for waiver-based Home and Community-
Based Service (HCBS) participants.
Completing appropriate documentation related to casework.
2. Nursing Facility Follow-up Phone Calls.
Conduct timely follow up phone calls to participants placed short term in nursing facility as identified by Choices for Care screen. Educate participants and caregivers on available services and supports as an option to successfully transition the individual home/back into the community.
3. Program Compliance and Quality Assurance.
Complete assigned work within contract, grant, and program requirements.
Develop person-centered skills and approaches with participants. Work toward the development of a quality Care Coordination Program (participation at meetings, identification of services gaps, etc.)
4. Professional Development.
Participate in activities and educational opportunities leading to own professional development (within and outside the agency).
5. All other duties as assigned.
Assist to assure smooth operations of services in alignment with agency mission and to complete other related tasks as assigned.
